The cost is $109 within 10 miles of zip code 91942. Appointments can often be made the same day as the examination.

Transit companies receive a discount when scheduling a minimum of 6 physicals on a pre-scheduled transit terminal visit, with no trip charge. Companies can schedule a standing monthly, bi-monthly, or quarterly visit.

Special pricing will be added for: out of area rest stop visits, after hours, and weekend appointments.
DOT physical examination certificates are typically valid for up to 24 months. The medical examiner my issue a certificate for less than 24 months for certain medical conditions.
Yes. If you have a history of epilepsy, insulin dependent diabetes, fail to meet minimum vision and hearing standards you are disqualified as a driver. Also, the use of Methadone, loss or impairment of a limb that interferes with commercial driving tasks will disqualify you. A history of alcoholism or use of narcotics, amphetamines, or other addictive drugs or medications without a prescription from a treating physician automatically disqualifies you. In addition, untreated sleep apnea, recent surgeries, current back injuries, and other conditions may prevent you from passing the physical exam. There are other conditions not listed here that may result in not being certified to drive the day of your examination.

  2. Bring a current valid driver license or passport.
  3. Bring corrective lenses or contacts with you (if you use them).
  4. Bring hearing aids with you (if you use them).
  5. Bring a COMPLETE list of ALL medications.
  6. Bring EACH of your doctor’s contact information including phone numbers, addresses, email addresses, and fax numbers.
  7. Bring a current printout of CPAP machine usage from your doctor.
  8. Bring any applicable clearance letters from your Primary Doctor or Specialist if needed.
  9. Be prepared to give a urine sample.
